Please complete this form and send with the following:
I. Your check, money order or cashiers check made out to: Anime Festival of Wichita. Please do not send cash.
2. The signed and completed forms.
3. Please fill in a complete list of wares even if you have been a past dealer. ( CD’s , Movies, Plushies, etc…) If you display items that are not on your list, you will be asked to remove them and no longer sell those items.
4. Please include your KS state sales tax number. (If you do more than two shows a year in Kansas you are required to have your own sales tax number.)
5. Three digital (may email) or printed photos of the wares you intend to sell
6. One digital (may email) or printed photo of your booth display.

THIS IS A JURIED EVENT AND ALL APPLICATIONS WILL BE ACCEPTED ON QUALITY, UNIQUENESS AND APPROPRIATENESS OF ALL PRODUCTS FOR SALE.

All application fees will be deposited upon receipt.
If your application is refused, your fee will be returned and you may resubmit.
Dealer Space: Standard 6 ft table along the wall $175 each maximum 5
                         Standard 6 foot table in center of room $150 each maximum 5
                     
Electric is a charge of  $15.
Tables are first come, first served, if accepted 
Deadline for dealer tables is June 1st,  2011.

There will be a fee added for checks returned for insufficient funds.

The jury process will begin on January 10th, and acceptance letters will be emailed out as soon as possible. You may be asked to limit or eliminate certain wares in order to be accepted. E-mail if you have any questions. If you are not accepted, we will mail you a refund check and a reason for denial.

All federal, state, and local laws governing retail sales tax must be followed. You are responsible for all federal, state, and local taxes. Kansas State sales tax is now 7.3%. You must understand the rules and regulations of the show and must comply. Failure to comply may result in expulsion from the show.

Anime Festival of Wichita Hold Harmless Agreement

I, (print) __________________________________ being either owner, authorized agent, or responsible party doing business as _________________________________ shall indemnify, hold free and harmless, assume liability for and defend the Anime Festival of Wichita, Wolfmoon Productions LLC, their affiliates, agents, employees, volunteers, officers and directors from any and all costs and expenses including but not limited to, attorney’s fees, reasonable investigative and discovery costs, court cost and all other such sums which the Anime Festival of Wichita, their affiliates, agents, employees, officers, and directors may pay or become obligated to pay on account of any, all and every demand for claim or assertion of liability, or any claim or action founded thereon, arising or alleged to have out of participation/attendance or selling of wares, during the Anime Festival of Wichita, sponsored by Wolfmoon Productions LLC, their affiliates, agents, employees, officers, and directors or by any act of omission by the Anime Festival of Wichita, Wolfmoon Productions LLC, their affiliates, agents, employees, officers and directors.
